Digital Integrated health and nursing care with IT-supported nursing consultation visit according to §37.3 SGB XI
Currently, about 70% of those in need of care in Germany are cared for by family members. This meets the wishes of the majority of older people to remain in their home environment as long as possible. However, this also involves some risks. People who are homecared do not receive care transfer sheets or care planning, it is usually the case in professional care. In general, the quality and continuity of care cannot be optimally ensured. In addition, relatives providing care are under great strain. In particular, excessive demands in the area of extended care, which includes the home organisation of the care situation, can contribute to a transfer to inpatient facilities.
In order to counter this problem, nursing relatives are required to receive nursing advice according to §37.3 SGB XI in the home setting to ensure proper nursing care from care level 2. However, there are currently no standardised or evaluated counselling instruments for care counselling visits and no structured guidelines for the visit. It is therefore heavily dependent on the level of knowledge of the nursing staff performing the visit.
The projekt INGE - integrate4care was supported by the European Regional Development Fund (EFRE). The aim of INGE, in co-operation with our project partners (smartQ , gewi-Institut für Gesundheitswirtschaft e.V. , Hauspflegeverein Solingen and Fraunhofer FIT ), is to optimise care consultation visits with the help of an IT-supported consultation assistant. The task of the chair is the requirements analysis using the user-centered design approach, as well as the implementation of usability and effectiveness analyses. The duration of INGE: 01.01.2020 - 31.12.2022.
